At Professional Home Health Care, we are dedicated to providing excellent care services for Central Ohio. Our goal is to assist in caring for each individual in a compassionate manner. We help to provide a smooth transition from hospital or other facility to caring for our patients in their homes. We will care for each individual to meet their health care needs through an inter-disciplinary team of physicians, nurses, STNA's, personal care attendants and therapists as needed. The health care team will work with each individual through implementation of the physician's plan of care. We will assist in empowering the patient and family members to provide daily care needs as much as possible and to enable them to reach independent living.
We believe that health care is a basic human right. It must be available, coordinated and provided in a comprehensive way, combined with other human services when appropriate. Home health care is an important part of the continuous health care system and it will be provided in the most cost-effective way possible. Professional Home Health Care and our staff shall operate and furnish services in compliance with all applicable federal, state, and local laws and regulations and disclosure and ownership information. The following federal, state, local laws and regulations we comply with are as follows: Professional licensure laws, Practice Standards, Americans with Disabilities Act, Occupational Safety and Health Standards, Medicare/Medicaid Regulations, Omnibus Budget Reconciliation Act of 1987, Equal Employment Opportunities Act (ADA), Title VI of the Civil Rights Act of 1964, Balanced Budget Act of 1997, Fair Labor Standards Act and Occupational Licensure Laws including Public health regulations relating to infectious disease. To the best of our knowledge, based on our Quality Improvement Program and professional personnel practices, the services we provide comply with acceptable professional standards and principles.

To maintain patients in their homes as long as possible and provide services in the least restrictive setting. To assist patients in using all restorative methods, tools, and procedures to return patients to their optimal level of function. To provide cost-competitive, quality services. To develop a relationship with referral sources to effectively meet our patients’ home care needs.To function as a liaison between our patients and available community resources and assist our patients in coping with their needs and problems.
